Free Mental Health Assessments

NextStep.Health provides free wellness assessments. Use these assessments to inform yourself on your wellness journey. The following assessments test for a few common mental health conditions that are treatable. Take the NextStep today!
Addiction (DAST)

The Addiction (DAST) Assessment will help determine if your use of alcohol or drugs is an area to address.

Postpartum Depression (EPDS)

The Postpartum Depression (EPDS) Assessment is for new and expecting mothers who began feeling overwhelming sadness after becoming pregnant or giving birth.

PTSD Assessment

The PTSD (Post Traumatic Stress Disorder) Assessment is for those who are bothered by a traumatic life event.

Adverse Childhood Experience (ACE)

Adverse Childhood Experience (ACE) Assessment is for individuals to determine if they may have had an adverse childhood experience.

Depression for Teens (PHQ-A)

The Depression (PHQ-A) Assessment is for young people who are concerned that their emotions, attention, or behaviors might be signs of a problem.

Anxiety (GAD-7)

The Anxiety (GAD-7) Assessment is for people who feel that worry and fear are affecting their ability to function day-to-day.

ADHD (NICHQ Vanderbilt Parent Assessment)

The ADHD (NICHQ Vanderbilt Parent Assessment) is for parents of children who may have trouble focusing or are hyperactive.

Depression (PHQ-9)

The Depression (PHQ-9) Assessment is for individuals who are feeling overwhelming sadness.

Psychosis Assessment

The Psychosis Assessment is for young people (age 12-35) who feel like their brain is playing tricks on them (seeing, hearing or believing things that don't seem real or quite right).

Bipolar (MDQ)

The Bipolar (MDQ) Assessment is for individuals who have mood swings - or unusual or extreme shifts in mood and energy.

Suicidal Behavior (C-SSRS)

The Suicidal Behavior (C-SSRS) Assessment is for individuals to determine if they are having suicidal thoughts.

After taking an assessment, you will be provided with personalized resources, information, and tools to understand and improve your mental health.

If your results indicate you may be experiencing symptoms of a mental health condition, consider sharing your results with someone you trust. NextStep.Health has providers which can give you a full assessment and talk about your options for how to feel better.
